Your employee ID (EID), birth month, and birth date are required to enter this survey. This is to ensure the privacy of each individual taking the survey and the integrity of the overall survey data.

Leidos employees can find their EID in their Deltek timesheet, their Prism profile, or in the Leidos HR system (Workday).

Leidos QTC employees can find their LQTC EID in UKG Pro under the “Employee Summary” section.

Leidos QTC Commercial Services employees can find their LQTC EID in Paylocity, HR & Payroll, under the “Custom Tab”.

Leidos Biomedical Research employees can find their Leidos Corporate Number found in CostPoint Employee Self-Service under the “People” section.
